Patient Materials

Back in the 90s I worked awhile in a hospital-based EAP (Employee Assistance Program). My supervisor there was known as the "Queen of Handouts." She had a handout to fit almost any problem area. I guess she rubbed off on me a bit. Still to this day, I enjoy making written materials available to patients when it's appropriate to their concerns. While this often happens in session, with me jumping up and going over to my lateral files, I now am finding that people often find materials on their own and bring up comments or questions in session regarding what they've found online at home or in the office.

In a similar way, I have new patient forms available to be downloaded, filled out, and brought with you to the initial assessment. This will avoid your having to come in early in order to fill out the forms in the waiting room.

All of these materials are in PDF format, the universally agreed best format for making print and graphics available on the Web. If you need a copy of Adobe reader to open these files, it can be downloaded free.
